This form is designed to collect information about your background, interests, academics, demonstrated leadership, and career plans. Your answers to these questions will be used only in connection with your questionnaire for the scholarship program and will be reviewed by a scholarship selection committee.
I. Applicant Information

AUTHORIZATION / CERTIFICATION
ASSOCIATES
Please read the following statements. You must be able to answer “Yes” to all of these statements as a whole in order to qualify for consideration in the scholarship.

- I understand that if I have an unsuccessful assessment, interview, or internship, I will not be eligible to receive the scholarship and/or have the scholarship renewed.
- I confirm that I am enrolled full-time for the upcoming academic year as indicated in this application as an undergraduate student at the institution selected.
- I confirm that I am majoring in, and will continue to major in, one of the qualifying disciplines as selected in the application.
- I confirm that my current CGPA meets the minimum of 3.2 or greater (unrounded, on a 4.0 scale).
- I understand that I must maintain a CGPA of 3.2 or greater (unrounded, on a 4.0 scale) in order to maintain eligibility for the scholarship.
- I understand that I must alert the scholarship contact and/or their designated administrator if my graduation date changes or if I choose to co-op.

- By signing and submitting this questionnaire, I (1) declare that I have read, understood, and accepted the statements set out above; (2) declare that the information that I provided through this application is complete and true; (3) give my consent to the processing of the information provided through this application and any other personal data I may provide separately in the matter and to the extent described through this process; (4) give my consent that the personal data that I provide through this process may be disclosed to other companies in Shell and to third-party organizations providing administration or other relevant services to the appropriate company in Shell; (5) authorize Shell to verify, or have verified on their behalf, all statements provided through this process and to make any necessary reference checks; (6) authorize my school to release any information requested in order to be considered for the scholarship submitted; and (7) give my consent for Shell to keep any information that I submit, relating to the scholarship program, in its databases until I notify them otherwise.
